Crumble raw beef into the cold water in a pot and stir until fully broken up (this gives the classic fine texture).

Add everything except the pasta and toppings; bring to a simmer.

Simmer uncovered 1 1/2 hours, stirring occasionally, until thickened. Skim fat.

Serve over spaghetti topped with cheddar (and onion or beans for four- and five-way).
